Give him or her the photo and say "remember me next life". Make sure it's digital, watermarked and protected to be uneditable.
Talk to him/her about it. And then distance yourself from them...not forever, but for a while so you can give that person some space to get over you.
And whatever you do, DO NOT PARADE A NEW DATE IN FRONT OF THEM! That could hurt them.
